During the final two years of school, Year 11 and 12 students participate in the Transition to Adult Life program. This program evolves with the needs of each student and incorporates customised employment, everyday living and social activities that develop the life and work skills all our young people need to feel successful and fulfilled.
Each student’s individual transition plan incorporates a flexible and evolving curriculum targeted to meet the needs and interests of the student and includes:
- Customised employment (e.g. micro and social enterprise, job exploration)
- Self-determination/advocacy (e.g. clear yes/no, making choices and I have something to say)
- Communication (e.g. established and accessible aided and unaided communication systems)
- Health and safety (e.g. protective behaviours, personal hygiene and wellbeing)
- Social connections and relationships
- Everyday living skills
- Community participation, Leisure and recreation
- Using transport (e.g. using your Smartrider)
Our Seniors’ Hub is based in a purpose built 4 x2 bedroom, fully equipped and accessible home on our school site. This Hub offers the opportunity for our young adults to experience their senior curriculum and to build on their everyday living skills within a real-life environment.
Transition to Adult Life is a process that involves intensive collaboration between school, parents and community service providers. When transitioning students into the next phase of their education and adult life, it is important that families are presented with a number of appropriate options and that they are empowered throughout the process.
At Castlereagh School, we endeavour to provide meaningful transition options that meet the needs of both families and the individual students. Each year we offer a bus-tour of disability service organisations, NDIS supported workshops, workplace learning options, individual planning meetings and access to a Seamless Transition Program.
Our Seamless Transition Program provides experience, care and support to assist students and their families through the decision-making process. The program enables selection of the options best suited to meet the future needs for their child.
